During that time, you are free to ask any questions you may have about hypnosis, hypnotherapy, and anything related to your situation or issue. This enables us to make the most of your time and money during your session. How Do I Schedule a Session?
The first thing you'll need to do is download the CLIENT INTAKE FORM form and fill it out. Once completed, send it to me via fax (770.644.2370) or email (purehypnosis@mac.com). I will contact you by phone within two business days to make an appointment. Please download and sign the CLIENT BILL OF RIGHTS as well. You can send this along with the intake form. If you have any questions about these forms, feel free to call. If you would like to speak with me prior to scheduling a session, you can reach me at 678.488.7362, M-F from 9:00am-7:00pm. I am currently accepting new clients, and in addition to normal business hours, I also offer select evening and weekend appointments. My schedule tends to fill up out about two weeks in advance, but occasionally a time will be available sooner. If you have travel plans or an emergency, let me know and I'll do my best to accomodate.
